Documents required
To ensure the safe and compliant transportation of your furry friend with Elpis, we require the following standard documentation to be provided prior to departure:
- Animal registration details (MICROCHIP);
- Health booklet;
- Animal information if not included in the booklet: breed, gender, age, and weight;
- Certificate of good health from a veterinarian;
- Identification document of the person delivering the animal and the recipient, along with their contact phone numbers and email addresses;
- Signature on the delivery confirmation form by the recipient, which helps protect the volunteer, the association, and/or the individual, but most importantly, your pet friend.
By signing the form, the adopter acknowledges the obligation to register the animal with the competent veterinary service; - Download and complete the Authorization for the Transfer of Live Animals form;
- Download and complete the Model A Declaration of origin and destination of animals.
